Overview
Home-based, leadership opportunity! This is a full-time position for experienced nurses (RN, LPN) with a background working with skilled nursing and rehabilitation facilities. The Care Coordinator is responsible for facilitating IDT meetings, utilization review, patient and medical record assessment. We are looking for nurses with MDS or utilization review experience within SNF and/or rehabilitation centers. We have openings in Union, Mercer and Hudson counties. Interviews this week - start date is January 9th!!
