Nurse Practitioner or Physician Assistant

Overview

Looking for an NP or PA to facilitate out-patient house calls in the south Chicago area. Candidate must be vaccinated against COVID-19. Required Skills and Experience: - 2 + years as a NP - Active Nurse Practitioner License - FNP or AGNP - Strong belief in the revolutionary nature of house calls - Willingness to work in a revolutionary environment that sometimes necessitates last minute problem solving and out of the box thinking - Technically savvy and comfortable using tools such as iPads for charting and HIPAA secure messaging apps for care coordination - Must have active state license and DEA - Senior Patient Care Experience Required What you need to know: - Typically see one to two patients an hour, with goals set to drive interpersonal relationships with patients and families - A dedicated driver provides transportation while you maintain a medical supply kit - Use of iPad for EMR charting - Operations support for prescription refills, lab follow up, and coordinated care management

Qualifications

• Bachelor's degree or higher
• Valid Drug Enforcement Administration (DEA) in Illinois (IL)
• Senior Care (1+ years)
• ANY of the following valid licenses/certifications:
  • Family Nurse Practitioner (FNP) in Illinois (IL)
  • Adult Gerontology Nurse Practitioner (AGNP) in Illinois (IL)
  • Physician Assistant-Certified (PA-C) in Illinois (IL)
• Are you vaccinated against COIVD-19
