Family Medicine Physician

Overview

Looking for a physician to facilitate the out-patient home health visits in the Chicago area. Candidate must be vaccinated against COVID-19. Required Skills and Experience: -State Licensed Physician: Board Certified or Board Eligible, preferably in Family Medicine or Internal Medicine. -Minimum of 2 years primary care experience. -Must be tech-savvy and be able to easily utilize an array of applications, a s. smartphone, tablet, and EMR. -Background Check/ Active medical license with check is required. -Ability to treat and manage the health and well-being of patients as it pertains to their wellness, urgent medical issues, and chronic illness with a goal of establishing a primary care relationship. -Strong work ethic with effective communication skills and a collaborative spirit. -Attention to detail, especially in medical documentation. -Clinical supervision of medical assistants when working together. -Work with Medical Directors and the Chief Medical Officer to identify opportunities to innovate and improve medical care. What you need to know: - Flexible scheduling - Typically see one to two patients an hour, with goals set to drive interpersonal relationships with patients and families - A dedicated driver provides transportation while you maintain a medical supply kit - Use of iPad for EMR charting - Support for prescription refills, lab follow up, and coordinated care management

Qualifications

• Doctorate degree
• Valid Drug Enforcement Administration (DEA) in Illinois (IL)
• 1+ years of experience in ANY of the following:
  • Home Health Care
  • Senior Care
• Are you Board Certified or Board Eligible?
• Valid Medical Doctor (MD) in Illinois (IL)
• Are you vaccinated against COVID-19?
