Your Role As Director of Social Work, you provider leadership to a team of social workers (MSW, LMSW, LCSW) that influence and ensure the best clinical care for patients. You will work remotely but will travel primarily in both Michigan and New York State. You leverage an integrated technology platform and are complimented by an entire team of professionals including registered nurses, nurse care managers, health coaches, dieticians, pharmacists, psychiatrists, and even cardiologists and podiatrists. Primary functions of this role: Lead the social work function organizationally, ensuring the needs of the social work team are met, and that team members have the resources required to effectively impact the patients they serve. Hold a leadership role on the plan side high risk membership review to follow a team-identified panel of patients until their needs have stabilized. Care interventions will primarily be delivered via phone, in the PCP office and/or in the home. This roll will partner with clinical leadership, including Chief Medical Officer(s), the VP of Clinical Programs, VP of Health Services, and the Regional Chief Behavioral Health Officer in development of social work policies and clinical protocols toward managing patient needs. Empower your team members, through mentorship and skills development, to support high risk, high need patients and their families and address barriers to their overall care and management of chronic conditions. Establish key working relationships across the communities’ we serve, including community based resources that play an instrumental role in navigating the social determinates of health for a patient. Provide clinical oversight to team members as they work with patients and caregivers to understand barriers to accessing appropriate care and assist in developing a care plan for chronic condition management and overall care. Support care team in executing care plans Help team members learn to meet patients where they are at - both physically and mentally, to support care. Provide support in the home, in facilities, or via phone based on patient need. Train your team on the art of conversations around goals of care, patient values, and Advance Care Planning Establish working relationships with external partners such as discharge planners, UM staff, insurance case managers, state agencies such as Medicaid and SNF case managers to facilitate care Qualifications: -Must have a master’s degree in Social Work and relevant experience -At least 5 years’ experience leading a team of social workers, and providing necessary supervision for licensure -Experience leading team-based care, excellent collaboration skills -Experience with integrated MSW in a PCP office or home-based setting -Must be outgoing and willing to cold-call and build relationships with community organizations and partners to assess social determinants of health, medical terminology, and working with a geriatric population -Exceptional capacity to multitask -Self-directed leadership with comfort during periods of ambiguity -Strong communication and teaching skills. -Daily access to a vehicle with valid driver’s license -Working Remotely with travel to markets will be required How You are Supported You will benefit from Honest’s exceptional total rewards package, including competitive base pay with bonuses, generous paid time off policy including 12 paid holidays per year, reimbursement for continuing medical education, 401k with 4% match, health, dental, and vision insurance. Qualifications

Required
  • Master's degree or higher
  • Valid Licensed Clinical Social Worker (LCSW) in New York (NY)
  • Valid Licensed Clinical Social Worker (LCSW) in Michigan (MI)
  • 5+ years of experience in ANY of the following:
    • Supervising
    • Leading a team of social workers
Preferred
  • Have you ever managed a team of social workers? If yes, how many and in what type of environment?
  • Describe your experience building community partnerships/resources, coalition organizing and generally building a database of medical resources for your clientele.
