RN Case Manager

Overview

The Richmond State Supported Living Center is accepting submissions for active agency nurses for the following position: RN with Case Manager experience. Location: Richmond State Supported Living Center 2100 Preston St #1797, Richmond, TX 77469 Current Open Shifts: (Subject to change) * Day Shift, M-F, 8a-5p Duties of a RN Case Manager * Use excellent computer skills (outlook, excel, word) to create reports in a highly organized fashion. * Actively participate in Interdisciplinary team (IDT) meetings using verbal and written communication skills. * Perform comprehensive head to toe assessment. * Review orders, consults and diagnostics for data analysis. * Analyze data to create quarterly and annual comprehensive reports within specific timeframes. * Develop and implement chronic/long term care plans using SMART goals. * Educate support staff on plan of care. Job Requirements for all candidates: *Psych experience is a must have! * Must be licensed RN and able to practice in Texas * Preferably 2+ years as a direct care nurse . Case Manager experience preferred. . Knowledge of nursing procedures and techniques and the principles and process used in providing patient care. . Ability to provide patient care and to prepare and maintain records, to communicate effectively and to train others. If your staffing agency does not provide the services requested or at the facility in need, no response is required. . Must be in Active status at agency. Here is our Timeline for hiring: * Approximate start date week of 07/18/2022. * Minimum 3-month contract. * All compliance must be met before candidate can start. No exceptions. Other: * Regular rates apply for all positions * No extra time off will be available during the holiday season including Christmas, and New Year's. * Must have valid photo ID and current American Heart Association CPR certification * Must have negative PPD results within last 12 months * COVID-19 testing with negative result required (we provide) * COVID-19 Vaccination Card if applicable. * Must be able to successfully pass a urine drug screen and background check (you provide) * Must be able to successfully pass an FBI fingerprint background check (we provide)

Qualifications

• Valid Registered Nurse (RN)
• Case Management (CM) (2+ years)
• psychiatric care (2+ years)
