Overview
Job Description/Duties/Roles: You will apply the nursing process to plan and implement the care of patients, including: · Patient education and continuum of care planning · Patient intake and clinic data entry · Triage and management of three (3) provider message baskets in the EHR: o External lab management o Therapy plan treatment follow-up with prior authorizations o Patient prescription renewal, clinical concerns, and symptom management · Meeting with new patients in clinic for education, instructions and assisting the provider as needed · Communication of pertinent information to the health care team · Collaboration in establishing health patient goals and evaluating progress to nurture outcomes · Continuing education through learning activities adhering to the Board of Nursing standards You will deliver an exceptional experience to patients by: - Going above and beyond in all circumstances to ensure members receive the best care - Assisting in the coordination and execution of member engagement activities leading up to the clinic launch to build deep and trusted relationships within the community - Driving new patient enrollment year-round by identifying, cultivating, and securing new patients through resident engagement activities - Planning, coordinating, and executing local events in the community to help reach our ideal patients (e.g. medically underserved seniors) - Coordinating with health plan agents and brokers to ensure prospective patients are enrolled in the best health plan for their needs and assigned to our organization in a timely manner - Following up with disengaged patients to assist them in receiving services or to assist them in finding a more appropriate setting for their primary care needs Requirements Passion for our organization's concept, vision and patient population Ability to work without direct supervision and perform tasks autonomously Minimum Education: Must be a graduate of an accredited nursing program Current State License to practice Basic Life Support (BLS) certification 3+ years of work experience as LPN or MA Primary Care, Family Practice, or Geriatric Experience Preferred US Work Authorization Required Prior experience with Athena (EHR) preferred Ability to collaborate with professional colleagues as necessary to provide quality care Ability to consistently present a friendly, welcoming, and professional public-facing appearance
Qualifications
- Associate degree or higher

ANY of the following valid licenses/certifications:
- Licensed Practical Nurse (LPN) in Tennessee (TN)
- Certified Medical Assistant (CMA) in Tennessee (TN)

3+ years of experience in ANY of the following:
- LPN
- Medical Assistant
- Valid American Red Cross Basic Life Support (ARC BLS)
