Overview
Our client is seeking a Primary Care Provider (NP or PA) to join their growing team in Memphis, TN. This role will be responsible for assisting residents (in low income, senior living communities) with the coordination of all their medical needs and social services. All patients reside in the senior living community. The Primary Care Provider will be responsible for providing top-notch care to patients, building strong relationships with them, and are required to participate in community events. Top 5 Skills Required: • 3+ years of professional experience as an Advanced Practitioner • Experience in Primary Care, Family Practice, or Geriatric Specialty • Master’s Degree and Certification by one of the Nurse Practitioner or Physician Assistant certifying bodies • DEA certification with no restrictions • Active TN Nurse Practitioner or Physician Assistant license Important to Know: • Position is onsite in Memphis, TN • Work schedule for Care Provider’s will be Monday – Friday, 8-5pm • No on-call or after hours (Core patient hours are 9:30-4:30pm) • The Care Provider's will work alongside/be supported by the Care Navigators
Qualifications
- Master's degree or higher

ANY of the following valid licenses/certifications:
- Nurse Practitioner (NP) in Tennessee (TN)
- Physician Assistant-Certified (PA-C) in Tennessee (TN)
- a Primary Care Advanced Practitioner role (3+ years)
- Do you have a DEA certification with no restrictions?
- Do you have prior experience with Athena (EMR)?
- How soon are you available to start a new role?
