Overview

Summary of Primary Function/Scope: Under supervision and in accordance with agency policies, provides medically prescribed therapy. Provides advisory service to other processional workers who share responsibility for patient care. Participates in the development of the physical therapy program. Functions require special skills and knowledge of community health principles and practice. Essential Responsibilities: Patient/Client Care • Performs physical therapy evaluations, procedures, and diagnostic tests on patients for whom these have been medically prescribed, in compliance with the Agency electronic Medical Record (EMR) • Formulates and establishes short term and long-term treatment, goals and plans. • Teaches appropriate treatment procedures and supportive activities to patients and family members. • May provide direct physical therapy treatment i.e., therapeutic exercises, massage, heat, etc. Instructs in use and care of cane, wheelchair, braces, crutches, prosthetic appliances, etc. • Physical Therapist will supervise and guide the Physical Therapy Assistant, in all aspects of patient treatment in accordance with the State Practice Act. They will communicate often to provide quality care for achievement of goals. • Translates all exercises into functional activities or ADL. • Confers periodically with attending physicians and other team members regarding patient progress including physical and emotional conditions and reactions to treatment. • Interprets to the patient and family the implication of the treatment consistent with the actions and orders of the physician. • Assists individuals and families to accept and adjust positively to physical, mental, and social limitations. • Arranges for out-patient physical therapy when indicated. • Demonstrates rehabilitation procedures and supportive activities on selected patients to other health team members responsible for care of the patient. • Is responsible for maintaining current progress notes and records on all patients in accordance with Agency policies, Medicare guidelines, and Conditions of Participation. • Is available for advisory and continuing education services to staff as indicated. • Participates in patient progress and discharge review, case record review, utilization review, and team conferences to facilitate continuity of care. • Maintaining comprehensive understanding and utilization of community health for patient care. Documentation • Ensures that documentation is comprehensive, complete, accurate and submitted on the required EMR system. • Ensures that documentation is submitted in accordance with InHome Therapy’s policy. • Ensures that documentation is compliant with InHome Therapy’s policy and procedure. Professional • Knows and supports laws, regulation and agency’s policy and procedure related to professional activity, accepts responsibility and is accountable for the same. • Demonstrates professional behavior. • Participates in InHome Therapy’s Orientation, In-service and Quality Assurance activities. • Cooperates with other members of the health care team to achieve goals and achieve and maintain standards of high quality. • Incorporates current professional skills and knowledge into professional practice. • Understands professional limitations and seeks and /or accepts assistance, as necessary. • Makes efficient and effective use of time. • Assists InHome Therapy with problem solving and provides InHome Therapy with information and documentation, as requested. Education/Experience: • Bachelor’s degree/Master’s degree from an CAPTE Accredited School, which meets the requirements of the NBCOT certification exam. • Doctorate degree from an CAPTE Accredited School, which meets the requirements of the NBCOT certification exam preferred. • Successful completion of National Physical Therapy Exam (NBTE). • Must be currently licensed by the State in which the services will be performed. • Must be willing to maintain all licensure/certification requirements, to practice in the State, and in the Home Health Care field. Physical Requirements: • Regularly required to stand, walk, sit, talk, and hear • Occasionally required to reach with hands and arms and stoop, kneel, crouch, or crawl. • Occasionally lift and/or move up to 50 pounds. • Specific vision abilities required by this job include close vision, distance vision, and depth perception. Qualifications

Required
  • Valid American Board of Physical Therapy Specialties (ABPTS) in Illinois (IL)
  • Valid Regular Driver License (DL) in Illinois (IL)
  • Did you successfully complete the National Physical Therapy Exam (NBTE)?
