Overview
This role is for Weatherford, Granbury, and surrounding areas. Summary of Primary Function/Scope: Professional member of a Home Health Team who provides Speech and Language Therapy and/or Dysphagia services in the patient’s home under the direction of a plan of care (POC) ordered by a Physician. Performance evaluations are performed by peers. Essential Responsibilities: ● Follows Physician orders to evaluate the patient’s Speech/Language/Dysphagia deficits and recommend appropriate corrective therapy. ● Establishes and develops a plan of care (POC). Make changes to the POC as appropriate. Conducts appropriate assessments, in alignment with State and Federal regulations. ● Discusses programs/clients progress with Physician, team members and caregivers as appropriate. ● Instructs family and health team personnel in methods of Speech and Language improvements. Document initial evaluation and on-going assessment according to IHT procedures. ● Participates in team conferences and discharge planning activities. ● Observes all IHT policies and procedures including HIPAA. ● Respects patient rights, cultural beliefs. ● Accepts only those assignments for which she/he is qualified. ● Communicates with the Clinical Director about any problems or concerns. ● Complies with state regulatory acts. ● Ensures that documentation is comprehensive, complete, accurate and submitted on the required EMS system, in a timely manner. Submission of visits need to be within 24 hours of the Evaluations and the treatment visit. ● Ensures that documentation is submitted in accordance with IHT policy and procedure. ● Ensures that documentation is dated and signed or countersigned with name and title. Education/Experience: ● Master’s degree from an accredited university. ● Has been granted a certificate of Clinical Competence in Speech/Language Pathology or Audiology by the American Speech and Hearing Association; or meets the equivalent education requirements and work experience necessary for such certification. ● Must be currently licensed by the State, in which the services will be performed. ● Must be willing to maintain all licenses and certifications/continuing education, to practice in the State, and in the Home Health Care field. Physical Requirements: ● Regularly required to stand, walk, sit, talk, and hear ● Occasionally required to reach with hands and arms and stoop, kneel, crouch, or crawl. ● Occasionally lift and/or move up to 50 pounds. ● Specific vision abilities required by this job include close vision, distance vision, and depth perception. Qualifications
- Master's degree or higher
- Valid Speech Pathology in Texas (TX)
- Valid Regular Driver License (DL) in Texas (TX)
