They will be seeing 2-4 patients a day when in the field. 2. This is a full-time, home health-based position with travel reimbursements provided. 3. Field/Territory: To Be Assigned within 2 Hour Driving Radius Additional Job Details: 1. Deliver evidence-based, timely care in a manner that reduces avoidable hospitalizations, maximizes quality of life, and puts patient health and satisfaction first. 2. Conducts assessments, which includes comprehensive annual wellness exams on patients. (Assessments will occur in-home.) 3. Prescribe medications, order tests, and collaborate with patient’s physician. 4. Documents items such as: chief complaint, all applicable diagnosis, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and plan. 5. Collaborate with care management RNs and SWs to ensure consistent, highly satisfying patient experience, accurate documentation, and effective care planning. 6. Complete all documentation and paperwork in a timely manner. 7. Maintains quality of care and productivity standards as defined by the practice. 8. Perform effectively, as reflected by improved patient quality outcomes, which will be measured and reported daily.

Qualifications

Required
  • Valid NP in Texas (TX)
  • Valid Drug Enforcement Administration (DEA)
  • NP (Independent Practice) (2+ years)
  • 2+ years of experience in ANY of the following:
    • Home Health Care
    • Hospice
  • Managed Care (2+ years)
  • Do you have experience with computer-based data management programs?
  • Are you fully vaccinated?
  • Do you have experience conducting annual wellness visits or similar comprehensive visits and appropriately charting HCCs; general knowledge of risk adjustment.
  • Are you seeking full-time?
  • The work schedule is M-F from 8 AM - 5 PM. Are you available and comfortable with this schedule?
  • You will be travelling within a two-hour travel radius from the main location within which you will be visiting patients in-home. 80%+ of the job is on the road visiting patients. Are you comfortable with this and willing to put in the road time?
