Overview

Your Role As a PMHNP, you influence and ensure the best clinical care for your patients. You leverage an integrated technology platform and are complimented by an entire team of professionals including registered nurses, nurse care managers, health coaches, dieticians, pharmacists, psychiatrists, social workers, and even cardiologists and podiatrists. We are transforming the practice of psychiatry by combining the systems-based and holistic framework of our clinical practice with our Behavioral Health team. We design, implement, and manage psychiatry services in primary care settings for a complex Medicare population requiring psychiatric care. Primary functions for the PMHNP include: •Comprehensive management of psychiatric disease states and comorbidities, including assessment, diagnosis, treatment, and consultation, on a longitudinal basis; minimizes the avoidable hospital admissions. •Transitions of care support via collaborative pathways with inpatient and post-acute care facilities •Partner with Honest’s care team to identify and eliminate social determinants of healthcare for your patients, including functional capacity, home environment, health literacy, housing instability, food insecurity, and caregiver support. and in-home support service in care planning. •Participate in ongoing scheduled consultation with an interdisciplinary team to monitor patient progress. •Make clinically appropriate recommendations related to medication and behavioral health treatment, effectively manages, or collaborates care across inpatient, clinic, and home setting. •Provide a valuable support resource to colleagues in our primary care practices. •Support patients through direct telephonic and video psychiatric care for patients with a higher level of behavioral health acuity. •Accurately identifies and documents patient clinical conditions and assigns appropriate disease codes; establishes holistic treatment plan for the respective conditions. •Cogently explains the company’s IDT resources and clinical programs to patients and families; guides patients toward quality-of-life decisions aligned with patient goals and values. Recognizes goal directed care in response to patient needs is an important component of managing psychiatric conditions. •Represents Honest commendably to patients, families, providers, and community. •Other duties, as assigned. If any of that experience is within an integrated health delivery system that is a plus! •You are willing to perform the above referenced job duties virtually but are available for onsite meetings.

  • PMHNP (3+ years)
