Nurse Practitioner - Palliative Care Position Type- Full Time Our company was created by the merger of leading senior healthcare providers. With a combined footprint reaching 26 states and over 1000 senior living facilities, our company innovates solutions for senior care delivery with a unique focus on prevention and care coordination. SUMMARY Our company has immediate openings for experienced Advanced Practice Providers interested in the inpatient hospital setting providing palliative care. This position will be 7 days on/7 days off consulting on palliative care patients and working closely with hospitalists and intensivists. ESSENTIAL DUTIES AND RESPONSIBILITIES - Our inpatient palliative care providers work directly with patients and their families on end of life matters and well as goals of care planning for the chronically ill - Functions independently to complete history and physical data on patients; performs complete physical exams; interprets and integrates data to determine appropriate diagnosis and treatment plan - Functions independently to perform quality of life discussions with patients and their families - Orders and interprets diagnostic and therapeutic tests relative to patient needs - Works directly with care coordinator to ensure ancillary services are in place and timely - Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities - Collaborates with physician in managing acute and long-term medical needs of patients - Maintains and reviews patient records, charts, and other pertinent information accurately and efficiently - Instructs and educates patient and family regarding medications and treatment instructions post discharge and for home environment - Individual must become proficient in exercising autonomy in medical decision making and providing a broad range of diagnostic and therapeutic services QUALIFICATIONS - Must hold current state RN and NP/PA licensure. - Must hold a current Board Certification in a Nursing Specialty/Field. - Must possess a minimum of a Masters Degree in Nursing from accredited Colleges or Universities. - Experience in nursing is required, and some experience as a Nurse Practitioner/Provider is preferred, but not necessary, although demonstrated critical thinking skills are required. - Must possess knowledge of Nursing and clinical Nurse Practitioner critical thinking skills application to the aged, chronically ill, and convalescent resident. - Strong leadership, interpersonal, and problem-solving skills preferred. - Must possess excellent written and oral communication skills - Must possess traits of punctuality, organizational skills, and ability to function within a team. Qualifications

Required
  • Master's degree or higher
  • Valid Nurse Practitioner (NP) in Texas (TX)
  • Valid Registered Nurse (RN) in Texas (TX)
  • Have you read and understand the attached information regarding Work Relative Value Units (RVUs)?
  • Valid Physician Assistant-Certified (PA-C) in Texas (TX)
