Experience working in a community clinic or non-profit Tell me about this job – The Integrated Behavioral Health program is a pilot program funded by Santa Clara County that launched in June of 2012 utilizing the IMPACT model to treat patients in the primary care setting within a multidisciplinary team including Primary Care Physician, Licensed Psychologist, Licensed Clinical Social Worker, Psychiatrist, and Peer Partner. The primary tasks of this position include providing mental health assessments and treatment to patients ages 6 and over. Additional duties include consulting with IBH providers to discuss patient care, referring patients to other mental health services or community resources, and maintaining electronic health records for patients.
