By leveraging technology and evidence-based care plans that are infused into a behavioral change framework, Our Company’s virtual CR program reduces barriers to care and effectively addresses patients’ physical and mental health needs from home. By extending CR services to eligible patients that hospital outpatient-based programs do not have capacity for, Our Company allows Health Systems to serve a broader patient base and gain a new profit center. Further, our clinical model has the flexibility to scale across all heart and vascular service lines and beyond, reducing mortality risk for the chronic disease patients who need this care the most! About the Role As Lead Nurse Practitioner and one of our first clinical team hires, you will be in a unique position to drive the development and execution of Our Company’s innovative clinical model. Your responsibilities will include providing direct and indirect oversight of virtual patient care, developing/reviewing clinical P&Ps, and driving Our Company’s clinical quality program. You will be a part of an integrated team of clinical professionals including Clinical Exercise Physiologists and Licensed Clinical Social Workers working together to deliver a world class patient experience. What you’ll do: ● Participate in the design and launch of Our Company’s comprehensive, evidence-based virtual-first cardiac recovery program in addition to providing direct patient-facing care and oversight (approximately 50/50 split of role) ● Lead Our Company’s clinical and operational quality assurance program, performing reviews of care team documentation, and lead continuous quality improvement initiatives ● Participate in the clinical hiring process, including vetting candidates, interviewing and onboarding training ● Work with the Head of Operations and Director of Clinical Programs to achieve key clinical success metrics across health system partner contracts ● Participate in health system partner strategy and business development discussions ● Act as clinical liaison with partner health systems for patient case reviews, escalations, etc. ● Make independent judgments and change to treatment according to professional and regulatory standards ● Communicate effectively across our integrated clinical team and our partner health systems, escalating unusual and emergent situations What you’ll bring to the table: Skills/Traits ○ An innovative mindset, and willingness to challenge status quo of traditional cardiac rehab experience ○ Experience in cardiac disease management, rehabilitation, and wellness in a clinical administrative leadership role ○ Experience in early-stage digital/virtual health startups and building clinical models from the ground up ○ At least 5 years of experience in clinical practice (hospital or clinic based) ○ A passion for 1:1 patient coaching and driving patient behavior change ○ Clinical experience in heart and vascular field ○ (Preferred) Understanding of relevant industry clinical practice guidelines and quality measures for Cardiac Rehabilitation (ex. AACVPR, ACSM, NB-HWC) ○ Understanding of relevant patient educational content used in Cardiac Rehab ○ Expertise using technology in cardiac rehabilitation (ex. Fitness trackers, remote blood pressure cuffs, weight scales, etc.) or a willingness to learn new technologies ○ Effective and empathetic communication skills across phone, video and asynchronous messaging ○ A willingness to travel at least once per quarter for in-person team meetings Education/Certifications/Licenses ○ Current and unrestricted Illinois state Nurse Practitioner license (willingness to obtain licensure in additional states) ○ Obtained full practice authority (minimum 4,000 clinical hours and 250 hours of continuing education) to practice independently without Physician supervision as per Illinois state regulations ○ Nationally board certified with maintenance of professional credentialing and CME standards ○ Active DEA Controlled Substance Registration ○ (Preferred) Masters of Business Administration or other relevant Masters level degree (ex. Masters of Public Health) ○ Basic Life Support (BLS) certification

  • Valid American Red Cross Basic Life Support (ARC BLS)

  • Valid Nurse Practitioner (NP)
  • Cardiology Care (5+ years)
  • Lead NP (3+ years)

  • Valid Nurse Practitioner (NP) in Illinois (IL)
  • Describe your experience working for a virtual business and/or digital chronic disease program.
  • Describe your leadership experience working for a Heathcare start-up.
  • Do you have full practice authority in the State of Illinois?
