By leveraging technology and evidence-based care plans that are infused into a behavioral change framework, Our Company’s virtual CR program reduces barriers to care and effectively addresses patients’ physical and mental health needs from home. By extending CR services to eligible patients that hospital outpatient-based programs do not have capacity for, Our Company allows Health Systems to serve a broader patient base and gain a new profit center. Further, our clinical model has the flexibility to scale across all heart and vascular service lines and beyond, reducing mortality risk for the chronic disease patients who need this care the most! About the Role As Clinical Exercise Physiologist and one of our first clinical team hires, you will be responsible for assessing patient cardiovascular health risk factors, developing/managing a personalized care plan tailored to their specific medical, physical, mental, and psychosocial needs, and coordinating their care across Our Company’s and the Health System’s care team. You will be a part of an integrated team of clinical professionals including Nurse Practitioners and Licensed Clinical Social Workers working together to deliver a world class patient experience. What you’ll do: ● Provide virtual cardiac recovery services to a panel of patients ● Evaluate patient functional abilities and limitations to determine appropriate interventions ● Design individualized treatment plans that consider the patient’s physical, mental, emotional and behavioral change needs and abilities. ● Ensure that treatment plans are supported by standard practices and/or current literature ● Follow plan of care to accomplish functions within established policies, procedures and scope of practice ● Re-evaluate treatment plans, as needed, to ensure safety and maximum patient benefit ● Promote patient engagement, and successfully achieve patient and program goals ● Make independent judgments and change to treatment according to professional and regulatory standards ● Coach patients using behavioral change techniques (ex. motivational interviewing) ● Communicate effectively across our integrated clinical team and our partner health systems, escalating unusual and emergent situations ● Provide complete, accurate and timely documentation of patient sessions What you’ll bring to the table: Skills/Traits ○ An innovative mindset, and willingness to challenge status quo of traditional cardiac rehab experience ○ Experience in cardiac disease management, rehabilitation, and wellness ○ Understanding of relevant industry clinical practice guidelines and quality measures for Cardiac Rehabilitation and Health and Wellness Coaching (ex. AACVPR, ACSM, NB-HWC) ○ Understanding of relevant patient educational content used in Cardiac Rehab ○ Expertise using technology in cardiac rehabilitation (ex. Fitness trackers, remote blood pressure cuffs, weight scales, etc.) or a willingness to learn new technologies ○ Effective and empathetic communication skills across phone, video and asynchronous messaging ○ A willingness to travel at least once per quarter for in-person team meetings Education/Certifications/Licenses ○ Master's degree or bachelor’s degree with equivalent work experience in Exercise Physiology, Exercise Science, Kinesiology, or a related field at an accredited institution ○ Minimum 2 years of clinical experience ○ AACVPR Certified Cardiac Rehabilitation Professional Certification or the ACSM Clinical Exercise Physiologist Certification prior to start (and willingness to obtain the other within 6 months of hiring) ○ Wellness coaching certification from a NBHWC accredited program ○ Basic Life Support (BLS) certification
Qualifications
- Master's degree or higher

- AACVPR Certified Cardiac Rehabilitation Professional Certification
- ACSM Clinical Exercise Physiologist Certification

2+ years of experience in ANY of the following:
- Epic
- Cerner
- Valid Wellness Coaching Certification from a NBHWC Accredited Program
- Valid American Red Cross Basic Life Support (ARC BLS)
- Cardiac Rehab (2+ years)
- Virtual Health/Telehealth (2+ years)
