Doctor of Medicine (MD)

Overview

This is a remote position. Provider can work from the comfort of their own home, as long as they are physically in the state of Florida. Provider needs to live within 75 miles or less from the client site - 777 SW 12th Avenue, Deerfield Beach, FL 33442 Dates & times of coverage needed: - March 15, 2023 – May 23, 2023 Type of agency staff​ needed: - MD, Physician - Primary Care, Occ Med, Radiology Health Center Name: - Publix (Lakeland & Deerfield) - Remote/Virtual Health Center Address: - TBD Number of patients seen per day: - X Scope of Services: - Primary Care, Occ Med, Radiology What other staff will be onsite at the health center: - NP, MAs, Rad Tech High level duties: - They’ll need to use their personal laptop - They’ll log on weekly or bi-weekly or monthly and review a % of patient charts - They will do this electronically through access to MILL - They will fill out a form indicating their review of each patient’s record and offer feedback for each, if applicable On-Call Details: Provider is expected to be “on call” during Publix working hours of Monday – Friday; 8a-5p EST. We will pay them a minimum of 2 hours a week just to be on call, even if no calls are taken. However, should they physically take calls, we will pay them for the hours worked. They will need to sign a state appropriate provider collaboration agreement and make sure they follow those guidelines, including ensuring they are not supervising more than they are allotted to for supervision.

Qualifications

• Doctorate degree
• Valid Medical Doctor (MD) in Florida (FL)
