Overview

LPN --Our clinic serves Granite School District employees ages 2 and up (we don’t see too many children, but there are some) and they would generally handle 8 – 10 patients on the 1 p.m. – 7 p.m. shift. --Sometimes this is less/more depending on the day. --We generally have one clinical support to one medical provider. --The candidate should have some experience in a clinical setting – phlebotomy, vaccines, CLIA tests; be able to work in a fast paced environment and keep up with busy schedules and needs of medical providers, possess critical thinking skills, and work cooperatively as a team member. --Attendance must be excellent. --Masks required for staff. EPIC (EMR) experience helpful, but not a pre-requisite. Shifts: M-F -- 1pm-7pm **Also occasional 8 am - 1 pm on Saturdays: 1 - 2 two times a month. **Possible temp to hire - would like to find the right candidate for a permanent position. Location: • West Valley Utah Required skills and experience: • Active LPN License in the state of UT • Experienced in Phlebotomy, vaccines, CLIA tests • Be able to work in a fast paced environment and keep up with busy schedules and needs of medical providers, possess critical thinking skills, and work cooperatively as a team member. • BLS Certification Interview process: • Quick interview process • One with Relode, One with hiring manager

Qualifications

• Are you vaccinated against COVID-19
• Have you had a PPD Skin Test with Negative results within the last 12 months? Must have a copy of the results, if you have not had the test, or do not have a copy of the results, please select NO.
• Have you been vaccinated against HepB or had a titers test drawn? Must have proof of vaccination or titers test to select YES.
• Valid Licensed Practical Nurse (LPN) in Utah (UT)
• ANY of the following valid licenses/certifications:
  • American Heart Association Basic Life Support (AHA BLS)
  • American Red Cross Basic Life Support (ARC BLS)
• 2+ years of experience in ANY of the following:
  • phlebotomy
  • Primary Care
  • annual exams
  • Epic
  • vaccine administration
  • Front Desk Admin
