Required
  • Valid Registered Nurse (RN) in New York (NY)
  • Do you have a valid drivers license?
  • 1+ years of experience in ANY of the following:
    • Hospice
    • Medical/Surgical (MedSurg)
  • Bachelor's degree or higher

Overview

What you need to know: - Minimum of one year of clinical nursing experience including at least six months of Hospice, or Med/Surg or ICU or Emergency Dep't or Home care, Palliative Care, Oncology or Long Term Care experience, required. - Will be working with end of life/hospice patients - Will be doing 2-3 in home hospice visits per day - Interview process is one Zoom/Teams video interview with the Clinical Field Manager or Branch Director - Will not hire new grads/new nurses with less than 12months of experience - Extensive 2-4 weeks of orientation - Monday - Friday schedule 8:30am-4:30pm (must also work one weekend per month) - Must have RN license in NY - Must have a car, a NY issued drivers license, and proper auto insurance - Parking and other expenses necessary will be reimbursed - $12,000 sign on bonus. $5000 will be paid out at the end of the first month and the remaining $7000 will be paid out after 12 months Salary Information - Base - $97,100 - $750 per year of clinical experience - Bachelors - $1700 - Masters - $1950 - Any nursing certification like wound care, pediatrics, psychiatrics, etc. - $1300 - Eligible for a 3-4% raise each year Union Nurse Information: - All field nurses are part of the UFT union (United Federation of Teachers - Federation of Nurses Chapter). - For their dues, As per UFT contract: - 1st check of the month: 0.85% of gross earnings plus $20.25 - 2nd check of the month: 0.85% of gross earnings plus $20.25 – and if total for the month is less than $63.81 – remainder is also deducted from this check. - Each nurse must be either already part of UFT union or not part of another union -If they're a member of another union they are not eligible for this position Benefits at a Glance: - Medical, Vision and Dental - Employee Assistance Program (EAP) - Health Care Account/Dependent Care Account - Commuter "TRIP" Benefit - Basic Life Insurance, Supplemental Group Life and AD&D Insurance - Short and Long Term Disability Benefits - Retirement Saving Plan, CARE Pension Plan - Paid Time Off (PTO) - Holidays - Paid Family Leave - Tuition Reimbursement - Health & Fitness Discounts, Employee Discounts - Credit Union - And much more In this Role you'll: - Assessments needs of the clients to develop a plan of care - Case Management within a Point System - Understanding and implementing proper protocols to determine billing source for patient/client care. - Consult with Management on complex/unique cases - Provide a plan of care with support of the Clinical/Medical/appropriate multidisciplinary members to ensure the services follow the plan of care by assessment, planning, coordination, counseling, teaching and supervision to meet needs of the clients/patients/. - Monitor/Assess patients/clients' condition, effects of the care plan, - Oversees/supervises the care provided to the patient by Licensed Practical Nurses and Paraprofessionals. - Manages, plans, coordinates and executes the patient’s safe discharge from service. - Ensures that physician’s orders, discharge summary and communication to team members, patient and family members adhere to VNS Health and regulatory requirements. - Communicates and consults with patient’s MD with all manners when needed, manage and coordinate all aspects of assigned caseload. - Assists patients with activities of daily living - Documents content and process of all services provided in internal system properly. - Transport/utilize medical equipment to and from clients/patients' home/facilities. Travel to patient/clients' homes and/or other facilities. - Perform other related duties and participates in special project, as assigned Licensure: - License and current registration to practice as a Registered Professional Nurse in New York State required. Education: - State approved Diploma in Nursing OR Associate's Degree in nursing with at least one year of experience as a full-time staff nurse in a medical-surgical nursing area or other clinical healthcare experience required OR LPN with at least one year of experience as a full-time staff nurse in a medical-surgical nursing area or other clinical healthcare experience required OR Bachelor's degree in nursing from a State-approved diploma program OR Bachelor's Degree in a health or human services field required.
