Type of agency staff needed: - Medical Assistant Dates & times of coverage needed: - M 8am – 5pm (w/a 1hr lunch) - T 9am – 7pm (w/a 1hr lunch) - W 8am – 5pm (w/a 1hr lunch) - Th 9am – 7pm (w/a 1hr lunch) - F 8am – 4:30pm (w/a 30 min lunch) Health Center Name: - Union Wellness Center, Crestwood Health Center Address: - 4736 Cal Sag Rd., Suite A - Crestwood, IL 60418 Date: - ASAP Number of patients seen per day: - 10-20 Scope of Services: - Primary Care, Urgent Care, Peds What other staff will be onsite at the health center: - Physician, RN, PT

Qualifications

• High School/GED
• Valid Certified Medical Assistant (CMA) in Illinois (IL)
• Direct Care (1+ years)
• Are you aware that a drug test must be taken before employment?
