Overview

Family Nurse Practitioner or PA 6 month contract** Job details: •Performs physical examinations and preventive health measures within prescribed guidelines and instructions of Physician. • Orders, interprets, and evaluates diagnostic tests to identify and assess patients’ clinical problems and health care needs. • Records physical findings, and formulates plan and prognosis, based on patient’s condition. Provides written home-going instructions. • Collaborates with Physician and other health professionals to prepare comprehensive patient care plan as necessary. • Prescribes or recommends medications or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures. Schedule: •M & W 8am-430p @ 200 Progressive Drive Ottoville, OH •Tues 8a-430 @ 200 Sugar Grove Lane Columbus Grove, OH •TH 6a-1130a @ 600 Crystal Ave Findlay, OH •F 530a-2p @ 2045 Industrial Drive Findlay, OH ***This could be a temp to hire position Location: Ottoville, OH Required skills and experience: • Active NP or PA License in the state of OH • 2+ years of Family Practice experience • Ability to work independently Interview process: • Quick interview process • One with Relode, One with hiring manager

Qualifications

• Are you vaccinated against COVID-19?
• Have you had a PPD Skin Test with Negative results within the last 12 months? Must have a copy of the results, if you have not had the test, or do not have a copy of the results, please select NO.
• Have you been vaccinated against HepB or had a titers test drawn? Must have proof of vaccination or titers test to select YES.
• ANY of the following valid licenses/certifications:
  • American Heart Association Basic Life Support (AHA BLS)
  • American Red Cross Basic Life Support (ARC BLS)
• Valid Nurse Practitioner (NP) in Ohio (OH)
• Are you able to work the full schedule and dates listed in the job description?

• Master's degree or higher
• 2+ years of experience in ANY of the following:
  • Nurse Practitioner
  • Family Practice
  • Occupational Health
