Overview

TYPE: - Nurse Practitioner (full-time, part-time, or traveler) - will onboard with client as a W2 employee. LOCATION: - Travel assignments within the State of Kentucky - although this job is says Louisville, we are accepting NP's from all across the State of KY SCHEDULE: - 40+ hours per week - Mon-Fri; 8am-5:30pm - No On-call or evenings - Must be willing to commute SALARY: - $114,500 - $129,500 - $1,500 Sign-On Bonus (eligible once they have completed 45 CHA’s within their first 90 days in the field) - Additional incentive based on completed assessments ($90 per) - Daily Per Diem and Travel Reimbursement BENEFITS: - Medical, Dental, Vision, 401K, PTO, Company paid holidays, Life Insurance and Mileage MUST HAVE: - Must be capable of traveling to & from each patients home. - Valid state driver license and proof of coverage. - Must be Covid Vaccinated. POSITION SUMMARY: The Nurse Practitioner (NP) brings expert care to members, through clinical assessments, screenings, and education. This role may provide care in a variety of settings including member/participant homes, assisted living facilities, Telehealth visits, and skilled nursing facilities. ESSENTIAL JOB FUNCTIONS - Perform screenings including medical history, physical exam, medication reconciliation and education. - Effectively identify, evaluate, and address the concerns of the member, employee or participant. - Administer and order point of care and lab based testing. - Order and interpret diagnostic imaging, laboratory results and provide clinically appropriate interventions, additional tests, or referral as needed. - Refer members to PCP, case management, disease management programs, and/or appropriate specialist as necessary. - Maintains positive control of all PHI information and compliance with OSHA and State Board of Nursing.

Qualifications

• Valid Nurse Practitioner (NP) in Kentucky (KY)
• Valid Physician Assistant-Certified (PA-C) in Kentucky (KY)
