Registered Nurse (RN) Case Manager

Overview

Our client is seeking a Registered Nurse (RN) Case Manager to join their growing team in Santa Maria, CA. This role will be responsible for participating in all phases of the Case Management Program to ensure continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance. Top Skills Required: • 1+ year of professional experience as an RN • Active, unrestricted RN License in the State of CA • BLS Certification (AHA or ARC) • Ability to pass a Government background check/security clearance • Associates Degree in Nursing • Possess one of the following certifications: - Commission for Case Manager Certification Certified Case Manager (CCM) - Certified Disability Management Specialist (CDMS) - Certified Rehabilitation Registered Nurse (CRRN) - Certified Occupational Health Nurse (COHN) - Certified Occupational Health Nurse-Specialist (COHN-S) - Advanced Certification in Continuity of Care (ACCC) - Certified Rehabilitation Counselor (CRC) - Nurses Credentialing Center Nurse Case Manager (RN-NCM) - Care Manager Certified (CMC) - must obtain within 14 months of employment - ERP will reimburse for the cost. • Associate degree or higher
• working as an RN (1+ years)
