COVID VACCINATION REQUIRED FOR THIS NEED. Candidate MUST be willing to work posted schedule Accommodations will NOT be considered! Shifts: Day (MWF) 6am-6pm LOCATION: 339 9th Ave La Grange, IL 60525. Patient Population: Adult and Geriatric EMR: Crown Web Dress Code: client will provide black scrub tops. Scrub bottoms: Personal scrubs of any color Requested time off during the course of this assignment MUST be noted and verified at upload. Traveler must be willing to work Labor Day holiday, including weekend before (If Needed/Scheduled). SUBMISSION REQUIREMENTS Candidate MUST be willing to work posted schedule. 1 year or more CURRENT Dialysis experience - Required Copy of Current, Valid Driver License - Required Geriatric patient experience - Required NXStage Dialysis Machine experience - REQUIRED, Please note experience on submission CNN or CDN Certification - PREFERRED Crown Web EMR experience - Preferred BSN - Preferred NXStage Dialysis Machine experience - REQUIRED CERTIFICATION REQUIREMENTS BLS (AHA) STATE LICENSE REQUIREMENTS Illinois

Qualifications

• Are you fully vaccinated against covid19?
• ANY of the following valid licenses/certifications:
  • Registered Nurse (RN) in Illinois (IL)
  • American Heart Association Basic Life Support (AHA BLS) in Illinois (IL)
  • Regular Driver License (DL) in Illinois (IL)
  • CNN or CDN in Illinois (IL)
• 2+ years of experience in ANY of the following:
  • Geriatric Care
  • Hospitalist
  • Dialysis Care
• Are you able to work all of the dates/times listed in the job description (including on call via phone)?
• Have you spoken to a Relode recruiter on the phone about all the job details?

• Bachelor's degree or higher
• Please share your most recent experience working as a Dialysis RN.
