What you need to know: - Monday - Friday 8am-5pm CST - Must be licensed in the state of TX Job Summary: As a Licensed Clinical Social Worker (LCSW) for our client, you will play a crucial role in our interdisciplinary team, providing telehealth services to Managed Long Term Support Services (MLTSS) patients. Your primary responsibilities will involve conducting comprehensive psychosocial assessments, delivering targeted interventions, and collaborating with our care management team to support the implementation of Person-Centered Support Plans (PCSP). Perks: Join a dynamic and innovative healthcare team Work remotely, enjoying a flexible schedule Make a positive impact on patients' lives Qualifications: Required: Master's degree in social work (MSW) Licensed as an LCSW in TX with an active NPI 3+ years' experience in medical social work; preference given for behavioral health experience Experience working with adult chronic disease patients and the geriatric population Ability to work and communicate effectively in a team environment Patient-centered focus with excellent communication skills Basic computer and EHR skills and understanding Medicare and Medicaid experience preferred Responsibilities: Conduct post-hospitalization assessments for select patients with mental health diagnoses. Respond promptly to referrals from the CareBridge team. Perform telephonic or telehealth psychosocial and economic assessments. Support individuals' needs through counseling, crisis intervention, and family therapy. Assess social determinants of health that may impact patients' well-being. Collaborate with the care management team, patients, and families to develop appropriate goals and interventions. Provide patient education on disease states, available services, and resources. Utilize motivational interviewing techniques to address non-adherence and quality of life concerns. Support medication compliance and assist with maintaining medical appointments. Facilitate patient access to community-based services and coordinate referrals as needed. Participate in interdisciplinary case conferences and document assessments and plans in the CareBridge system. Maintain excellent punctuality and attendance during work hours.
