NP - Primary Care

Overview

NP Primary Care or Urgent Care • Family Medicine, ages 2+. • Template Build allows for 1hr lunch and 1hr admin time. • 30 min office visits with 45 min physical and 15min telephone encounters. • Epic EHR system. • Clinic also has PT, Chiro, Behavioral Health and Vision. • On site provider dispensing for medications. • Union Sponsored Health Care, billing does not go through insurance. • Collaborative oversite per client's policy • MA clinical assistance with RN Health Center Manager onsite. Initial onboarding/training would require 2-3 weeks. M,T,W,F - 8:00-5:00pm Th- 10:00-7:00pm Schedule: M,T,W,F - 8:00-5:00pm Th- 10:00-7:00pm Template Build allows for 1hr lunch and 1hr admin time Location: 4979 Indiana Ave Lisle, IL 60532 Required skills and experience: • Active NP license in IL • DEA license is required. • Preferred candidate would have 3+ years Primary Care and chronic condition management experience • Epic experience. Qualifications

• Are you vaccinated against COVID-19?
• Have you had a PPD Skin Test with Negative results within the last 12 months? Must have a copy of the results, if you have not had the test, or do not have a copy of the results, please select NO.
• Have you been vaccinated against HepB or had a titers test drawn? Must have proof of vaccination or titers test to select YES.
• ANY of the following valid licenses/certifications:
  • American Heart Association Basic Life Support (AHA BLS)
  • American Red Cross Basic Life Support (ARC BLS)
  • Drug Enforcement Administration (DEA)
• Valid Nurse Practitioner (NP) in Illinois (IL)
• Are you able to work the full schedule and dates listed in the job description?

• Master's degree or higher
• 3+ years of experience in ANY of the following:
  • Primary Care
  • Epic
  • Urgent Care
  • chronic condition management
