SLP - Home Health LOCATION Kāne‘ohe HI JOB TYPE Temp OPENINGS 1 DESCRIPTION Provides speech therapy services to patients with speech, language or swallowing disorders. Productivity expectation is 25 points per week. --Assesses and documents speech therapy data related to patient assessments, patient care needs, interventions and outcomes. --Identifies changes and trends throughout physical and emotional parameters. --Reviews treatment plans and assesses individual performances to modify, change or write new programs. --Maintains documentation as required by law, establishment policy and administrative regulation. --Instructs patients on ways to monitor speech and practice new skills. SUBMISSION REQUIREMENTS Bachelor's Degree - REQUIRED 1+ year Home Health experience - REQUIRED Travel experience - REQUIRED Active and clear driver's license and car insurance - REQUIRED OASIS experience - REQUIRED COVID and FLU vaccine - REQUIRED. Will not take declinations! Mileage reimbursed at .43 cents per mile (lower than IRS established rate) CERTIFICATION REQUIREMENTS BLS STATE LICENSE REQUIREMENTS Hawaii ADDITIONAL LICENSE REQUIREMENTS Speech Pathologist licensure in the state of practice: Required Will not be assigned a territory until close to start date as their needs are changing DURATION DESCRIPTION 13 weeks SHIFT NOTES Hours worked per day may vary, depending on scheduling and patient need - 24 hours GTD WEEKEND REQUIREMENTS No Weekends

Qualifications

• Bachelor's degree or higher
• 1+ years of experience in ANY of the following:
  • SLP
  • Home Health Care
  • OASIS
• ANY of the following valid licenses/certifications:
  • SLP in Hawaii (HI)
  • Regular Driver License (DL) in Hawaii (HI)
• Are you fully vaccinated against covid19?
• Are you up to date on your Flu vaccine?
• Do you have travel experience working as an SLP?
• Have you spoken on the phone with a Relode recruiter about this position?
