Overview

Physician - Geriatric Medicine (Perm) LOCATION Philadelphia PA SUBMISSION REQUIREMENTS ALL OF THE FOLLOWING ARE REQUIRED AT NAME CLEAR UNLESS NOTED OTHERWISE. Board Certified in Internal Medicine or Family Medicine - REQUIRED Board Certified in Geriatric Medicine - PREFERRED Active PA license - REQUIRED CDS Certification - REQUIRED DEA Registration - REQUIRED Availability to begin working and state interest in Perm - REQUIRED Minimum 1 year working with the frail or elderly population - REQUIRED BLS - REQUIRED Clean/Clear malpractice - HIGHLY PREFERRED EMR TruChart experience - HIGHLY PREFERRED *Please note: •Minimum of 1 year experience working with the frail or elderly population is required (no new grads) •Not accepting visa candidates •Provider must be interested in Perm (not accepting Locums) Job Description: •Shift is Monday - Friday 8:00a - 4:30p with on call requirements.  Call is evenly distributed among all providers, approximately 5 to 7 days per month as is chosen by the provider. •Provide primary medical care to participants, including assessment, development of a plan of care in collaboration with the interdisciplinary team, provision of direct participant medical care, and evaluation of effectiveness of the plan of care •Function in a collegial relationship with the other health care professionals, making independent decisions regarding medical needs and health regimens, while carrying out medical procedures as indicated. •Refer participants to medical specialist as indicated.  •Manages all care of participants in the nursing home: Provides regular visits to manage the care of participants in the nursing home in collaboration with the Interdisciplinary Team; maintains communication with nursing home staff for continuous monitoring of PACE participants on a case-by-case basis; admits nursing home participants to hospital when necessary.  Oversees and manages participant’s use of medical specialist and inpatient care.  Communicates and follows up with specialist regarding outcome of visit. •Complete documentation and uses appropriate diagnostic coding.
