  • Nurse Practitioner (5+ years)
  • 2+ years of experience in ANY of the following:

Overview

NP-Geriatric Medicine LOCATION Philadelphia PA JOB TYPE Perm Schedule: M-F No weekends -On call rotation is possible Job Description: •The primary purpose of your job position is to provide primary medical management in collaboration with the PACE Organization (PO) Physician(s).  •Performs assessments in collaboration with the physician and as needed.  •Develops and implements appropriate plans of care to PO participants.  •Evaluates participant physical complaints and provides appropriate treatment.  •Provides participants and caregiver teaching and education.  •Functions as a member of the Interdisciplinary Team (IDT).  •Provides daily oversight of PACE Organizations Clinic.  •Perform rounds in the POs contracted nursing homes.  •Regularly informs the IDT of the medical condition of each participant, remaining alert to pertinent input from other team members, participant’s caregivers, as well as documenting changes in a participant’s medical record consistent with documentation policies. SUBMISSION REQUIREMENTS ALL OF THE FOLLOWING ARE REQUIRED AT NAME CLEAR UNLESS NOTED OTHERWISE.
