Physician Pod Leader

Overview

Required skills and experience: • Experience in Geriatrics, Home Care or complex patients • Must have experience treating patients with chronic illnesses • Home health experience • Active/unrestricted medical license and DEA • Board Certified or Board Eligible • EMR experience • Comfortable treating patients in their homes. • Must have exceptional communication skills and willingness to lead and build a team. • Must have the ability to lead and partner with NPs and support them with high risk patient cases. • Comfortable treating patients in their homes and traveling out in the field with an assigned MA. (**Client will provide a company car to use). What you need to know: The Physician Team Leader (Pod Leader) acts as a resource/coach impacting clinical performance of the team. The leader will collaborate with advance practice providers and build a collaborative team. We are seeking Physicians who desire the opportunity and experience of delivering quality and compassionate healthcare - within proven care models – to elderly individuals and those with complex medical issues, who are the forefront of everything we do. Qualifications

• Doctorate degree
• Valid Medical Doctor (MD) in Texas (TX)
• ANY of the following valid licenses/certifications:
  • Board Eligible
  • Board Certified
  • Drug Enforcement Administration (DEA)
• 3+ years of experience in ANY of the following:
  • Home Care
  • Geriatric/LTC
  • Urgent Care
  • Chronic Illnesses
