· What is the worksite location(s) for this position (city/town, hospital and/or clinic)? Gettysburg Hospital and possibly at Hanover Surgery Center · What is the reason for needing locums coverage? Vacant position · How many locum providers do you anticipate needing? 1 · Will case logs be required for privileging? · What is the anticipated start date? **Please note credentialing process can take 90 days and would prefer a start date to coincide with realistic credentialing timeframe. Likely starting early February to allow for credentialing. Credentialing time frame which can take up to 120 days. · What is the length of assignment needed? 6 months with possible extension · Are there specific dates needed of the locum provider or will you work around the locum provider availability? (Specific dates needed are preferred if possible): Overall need is not for a specific timeframe and can work around locum provider availability · What is the work schedule for this position? (days needed, hours or shift) Monday to Friday, 8 hour days · Describe the daily shift requirements(Productive, Night Call Coverage, 24hr Call Coverage, Weekday/Weeknight): Shifts are 8 hour days 7a – 3:30 p · What type of cases should the CRNA expect? Typical bread and butter of ortho, general, gyn, urology, robotics. This is a small local hospital · Are the CRNA’s supervised by the MD’s? What is the MD/CRNA ratio? 1 MD to 3 CRNAs usually · Do the CRNA’s prep the patients for the cases? · What is the average daily patient load? 5-7 · List any special certifications (such as ACLS) or requirements (including special procedures) needed to perform in this position. (ACLS, BLS , PALS) · Describe the make-up of support staff (techs, nursing, mid-levels, etc.) · Number of beds in your facility? There are (4) OR’s at Gettysburg Hospital · Willing to review clinicians with pending state license? ACTIVE PA LICENSE UNLESS OTHERWISE SPECIFIED. · Scrub colors / dress code: Blue and are provided · EMR: EPIC Location: 45 Monument Rd, York, PA 17403

Qualifications

• Valid Certified Registered Nurse Anesthetist (CRNA) in Pennsylvania (PA)
• Valid American Heart Association Advanced Cardiac Life Support (AHA ACLS)
• Valid American Heart Association Basic Life Support (AHA BLS)
• Valid American Heart Association Pediatric Advanced Life Support (AHA PALS)
