Speech Therapist
Overview
The Speech Therapist (Pathologist) will provide speech therapy services to Agency patients as prescribed by the attending physician and for providing Agency staff, and the patient’s physician with speech therapy consultation, as needed. The role involves assessing, planning, and implementing rehabilitation services for speech, language, and/or swallowing disorders, as well as providing evaluation of speech and language abilities through diagnostic testing. The Speech Therapist will also be responsible for recording and reporting the client’s progress in treatment and any change in patient’s condition to the physician, instructing family members and members of the health care team in methods of assisting the patient in improving communication skills, and maintaining adequate clinical and progress notes. Key Responsibilities ● Assess, plan, and implement rehabilitation services for speech, language, and/or swallowing disorders. ● Provide evaluation of speech and language abilities through diagnostic testing. Establish goals and estimate prognosis for recovering. ● Record and reports the client’s progress in treatment and any change in patient’s condition to the physician. ● Instruct family member(s) and members of the health care team in methods of assisting the patient in improving communication skills. ● Re-evaluate patient’s progress and the established rehabilitation program as often as necessary. ● Prepare periodic reports, as required by the Agency, which are sent to the physician and third party payers regarding care being provided. ● Assist in making appropriate referrals to outside audiology speech clinics or other community services as indicated. ● Maintain adequate clinical and progress notes and submits all required paperwork in a timely manner according to Agency policy. ● Provide liaison contact and follow-up with other community agencies. ● Participate in in-service, case conferences, and staff meetings as required. ● Maintain and updates professional competency through in-service attendance and similar professional resources. ● All other duties as assigned by supervisor. Schedule and Shift Details ● Employee will be required to drive daily for meetings and/or visits to residential homes. Employee will work in varied residential environments based on assigned caseload. Travel ● Employee travels by automobile and is exposed to changing weather conditions.
Qualifications
- Master's degree or higher

1+ years of experience in ANY of the following:
- Home Health Care
- Epic Home Health
- Home Health

ANY of the following valid licenses/certifications:
- American Heart Association Basic Life Support (AHA BLS)
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)
- American Heart Association Pediatric Advanced Life Support (AHA PALS)
- Valid SLP in Illinois (IL)
- Supervised Post-Certification in an institutional setting? (1+ years)
