Qualifications

Required
  • Master's degree or higher
  • 1+ years of experience in ANY of the following:
    • Physical Medicine & Rehab (PMR)
    • Sports Medicine
  • 1+ years of experience in ANY of the following:
    • Home Health Care
    • Epic Home Health
    • Home Health
  • Valid Physicial Therapist in Illinois (IL)
  • Have you applied to or worked for Residential Home Health & Hospice anytime in the past 6 months?
  • Have you spoken with your Relode recruiter about this position?
  • Valid Regular Driver License (DL)

Overview

As a Physical Therapist with our client, you will be responsible for providing Physical Therapy services to patients in their homes. You will assess patient functioning levels, develop treatment programs, and collaborate with the healthcare team to improve patient independence and physical functioning. This role requires strong interpersonal and communication skills, as well as the ability to maintain professional competency through ongoing education and resources. Key Responsibilities ● Perform Physical Therapy assessments of patient functioning levels and develop treatment programs ● Apply diagnostic and prognostic tests for muscle, nerve, joint, and functional ability ● Treat patients to relieve pain and restore function ● Make arrangements for outpatient services when necessary ● Record and report patient's reaction to treatment and any changes in condition ● Instruct patients and their families in the use of prosthetic, orthotic, and assistive devices ● Supervise physical therapy assistant and home health aide ● Prepare periodic reports and participate in in-service, case conferences, and staff meetings ● Maintain professional competency through in-service attendance and similar resources ● Prepare assessments, evaluations, summaries, and clinical/progress notes Schedule and Shift Details ● Monday to Friday, 8:00 AM to 5:00 PM Travel ● Occasional local travel
