Overview
Our client is seeking a Home Health Registered Nurse Case Manager to join their extraordinary care team. This position supports patients in Shippensburg, PA and surrounding areas. Key Responsibilities ● Lead and collaborate to provide management and delivery of patient care plans ● Support patients in Shippensburg, PA and surrounding areas ● Eligible for a $15K first year success bonus to reward the accomplishment of key achievements during the first year Schedule and Shift Details ● Mon-Fri 8am-5pm; on-call rotation is only 1 weekend every 4-8 weeks depending on staffing. When it is their scheduled weekend, they are on-call Saturday & Sunday, 8am-5pm only for SOC visits. They do not do on-call during the week or after hours. Travel ● Local travel requirements
Qualifications
- Associate degree or higher

1+ years of experience in ANY of the following:
- Electronic Medical Record (EMR)
- Other EMR
- TruChart EHR
- Valid Registered Nurse (RN) in Pennsylvania (PA)

ANY of the following valid licenses/certifications:
- American Heart Association Basic Life Support (AHA BLS)
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)
- RN (1+ years)
- Do you have a valid Driver's License, Reliable Mode of Transportation, and Auto Insurance (all 3)?
- Have you spoke with a Relode Recruiter about this role (via phone)?
