  • Valid American Heart Association Basic Life Support (AHA BLS)

Overview

Our client is seeking a full-time RN Case Manager for Home Health in West Cuyahoga County, OH. The ideal candidate will provide skilled nursing care, demonstrate effective communication and collaboration, and maintain patient safety. This role requires strong organizational skills, the ability to work independently, and a commitment to maintaining confidentiality. The candidate should have experience in home health care. This position will service the following coverage areas; Parma, Middleburg, Brooklyn, Brookpark, Fairview Park, Westpark and surrounding areas. Key Responsibilities ● Provide direct or indirect skilled comprehensive nursing care to a specified (or variety) of age and diagnostic groups ● Utilize principles of the nursing process to assist in data collection, planning, and evaluation of nursing care ● Demonstrate nursing knowledge and skills in the implementation of prevention, therapeutic, and rehabilitative nursing care ● Utilize effective interviewing techniques; sets limits, and collects data based on specific role ● Communicate and collaborate effectively with health care providers, third parties, patients, and families based on specific role ● Maintain awareness of comfort and safety needs of patients ● Observe patients, records significant conditions and reactions, and notify supervisor or Physician of patient's condition and reaction to drugs, treatments, and significant incidents ● Respond to life-saving situations based upon nursing standards, policies, procedures, and protocol ● Use oral and written communications to convey pertinent information to members of the healthcare team in a timely manner ● EMR: Home Care Home Base ● Equipment Provided: Tablet, Phone Schedule and Shift Details ● 8:30a-4:30p, 1 weekend per month ● Including working one summer and one winter holiday Benefits ● Medical, Dental, Vision Insurance ● 401k ● HSA/FSA ● PTO
