RN Home Health Intake Coordinator

Overview

As an RN Home Care Intake Coordinator, you will be responsible for assessing patients’ home health care needs, coordinating referrals to home care and hospice, and ensuring the best patient outcomes. You will play a vital role in providing compassionate care and promoting continuity of care for our clients. This position is located in Hamilton County, NJ. Key Responsibilities ● Assess and evaluate the needs of patients with multiple disease processes and comorbidities and develop an appropriate plan of care ● Provides education and strategies for patients and families ● Coordinates referrals to home care to promote continuity of care ● Partners with other clinicians and external providers to execute best patient outcomes ● Documents all changes in patient’s condition and updates care plans and outcomes ● Observe patients, records significant conditions and reactions, and notify supervisor or Physician of patient's condition and reaction to drugs, treatments, and significant incidents ● Use oral and written communications to convey pertinent information to members of the healthcare team in a timely manner Schedule and Shift Details ● 8:30a-4:30p, 1 weekend per month ● Including working one summer and one winter holiday Benefits ● Medical, Dental, Vision Insurance ● 401k ● HSA/FSA ● PTO

Qualifications

• Associate degree or higher
• Valid Registered Nurse (RN) in New Jersey (NJ)
• Valid American Heart Association Basic Life Support (AHA BLS)
