Speech Language Pathologist - Home Health

Overview

As a Speech Language Pathologist at our client, you will be responsible for providing speech therapy services to homebound patients in the Eastern Cuyahoga, OH territory. You will work independently with minimal supervision, collaborating with a diverse team to develop and implement care plans. This role offers the opportunity to make a meaningful impact on the lives of patients and their families. Key Responsibilities ● Perform initial assessment or admission to agency to determine treatment needs of patient within home environment on physical referral only ● Develop a plan of care with measurable goals and specific discipline related treatments in conjunction with physician referral/orders ● Provide direct speech therapy services as outlined in the plan of care including performance of speech therapy procedures, instruction of patient and or caregiver, recommendations for equipment or adaptation of home environment ● Re-evaluate and revise plan of care as needed with physician approval ● Perform scheduling and record keeping procedures in accordance with agency policy ● Perform other related duties as assigned Schedule and Shift Details ● 8a-4:30p, 1 weekend per month and rotation of all national holidays Benefits ● Medical, Dental, Vision Insurance ● 401k ● HSA/FSA ● PTO

Qualifications

• Master's degree or higher
• ALL of the following valid licenses/certifications:
  • American Heart Association Basic Life Support (AHA BLS)
  • Speech Language Pathology in Ohio (OH)
