Overview
Our client is seeking a LVN - Home Care Coordinator to develop and implement home care services for PACE program participants in Los Angeles, California. The ideal candidate will have a passion for serving high-risk seniors and frail older adults, possess an LVN License, and have experience in home care services management. Also including the coordination of all Durable Medical Equipment and community-based care. You are a collaborative team member who works as part of the Interdisciplinary Team to create a care plan to help participants’ meet their goals. This is primarily a center-based role. Key Responsibilities ● Coordinate home care services for PACE program participants ● Participate as a member of the Interdisciplinary Team (IDT) ● Conduct nursing care within LVN Scope of practice ● Provide general nursing care in the clinic and participant’s home ● Document in the EMR within set documentation timeline ● Communicate with home care employees to understand participant’s condition ● Collaborate with the Transportation Coordinator for participant transportation ● Schedule Home Care appointments and support the scheduler for other appointment needs ● Oversee visits completed by external agencies Schedule and Shift Details - 40 hours per week, Monday-Friday. Qualifications
- Associate degree or higher

3+ years of experience in ANY of the following:
- Home Health
- Geriatric Care

ALL of the following valid licenses/certifications:
- Licensed Vocational Nurse (LVN) in California (CA)
- American Heart Association Basic Life Support (AHA BLS)
- Have you worked or applied at myPlace Health in the last 12 months?
