Physical Therapist - Home Health

Overview

Our client is seeking a full-time Physical Therapist in Ocean County, NJ to provide home care therapy to individuals, helping restore function, improve mobility, and relieve pain. The ideal candidate will have a Bachelor’s Degree in Physical Therapy, one year of experience in a homecare setting, and the ability to work well under pressure with strong attention to detail. This role offers a dependable and compassionate work environment with opportunities for gaining experience and becoming an invaluable resource to our client. Key Responsibilities ● Perform initial assessment or admission to agency to determine patient treatment needs ● Develop a plan of care with measurable goals and specific discipline related treatments ● Provide direct physical therapy services as outlined in the plan of care ● Re-evaluate and revise plan of care as needed with physician approval ● Facilitate total care services to patients by making appropriate referrals to other disciplines ● Submit all documents in a timely manner ● Obtain physician approval to modify a current plan of care or treatment program ● Perform other related duties as assigned Schedule and Shift Details ● 8:30a-4:30p, 1 weekend per month and rotation of all national holidays. Qualifications

• Bachelor's degree or higher
• ALL of the following valid licenses/certifications:
  • American Heart Association Basic Life Support (AHA BLS)
  • Physical Therapy in New Jersey (NJ)
• Home Health (1+ years)
