Overview
As a Nurse Practitioner, you will be responsible for delivering high-quality care to homebound patients, promoting their well-being, and maintaining positive relationships with colleagues. This role requires a Master's degree in Nursing, relevant certifications, and strong communication and organizational skills. This positions supports the locations of Monmouth/Freehold County. This position is eligible to be part-time (3 days/wk) or full-time. Key Responsibilities ● Ensure delivery of high-quality care practices in the treatment of symptoms for homebound patients and their families ● Assess, diagnose, and develop appropriate care plans for homebound patients ● Monitor patient's condition and treatment responsiveness ● Act as agency representative with patients, families, and health agencies ● Adhere to infection control and safety policies ● Adhere to organization's policies on absenteeism and appearance ● EMR: Home Care Home Base ● Equipment Provided: Tablet, Phone Schedule and Shift Details ● M-F 8:30a-4:30p, no weekends ● Open to full-time or part-time opportunities This position starts at an hourly rate for the first 2-3 months and then changes to a PPV model. Qualifications
- Master's degree or higher

ALL of the following valid licenses/certifications:
- Adult Gerontology Nurse Practitioner (AGNP) in New Jersey (NJ)
- Controlled Dangerous Substances (CDS)
- Drug Enforcement Administration (DEA)
- American Heart Association Basic Life Support (AHA BLS)

2+ years of experience in ALL of the following:
- Nurse Practitioner
- Geriatric Care
