Overview
Join our client's team as an Enrollment Registered Nurse for the PACE Program in Los Angeles. This individual will help to bring the PACE (Program of All-Inclusive Care for the Elderly) program to serve frail elders and disabled older adults in underserved communities across Los Angeles. The Enrollment Registered Nurse is a key player of the Outreach Team as we find and assess prospective participants for our PACE program. This person acts as a liaison between the IDT (Interdisciplinary Team) and the Outreach Team to ensure a smooth enrollment process for the prospective participants. Key Responsibilities ● Collaborate with Outreach Specialist and IDT to ensure successful enrollment ● Conduct home visits to evaluate the prospective participant for potential eligibility for nursing home level of care ● Conduct home safety evaluations of the prospective participants’ home ● Participate in Enrollment Eligibility meetings to review Level of Care recommendations ● Complete Nursing Facility Level of Care (NFLOC) assessments and submit to DHCS for review; update/revise the assessment as requested by DHCS ● Maintain accurate documentation and record all evaluations in CRM software ● Participate in all PACE Audits by DHCS and CMS ● Support the Center Director to develop participant retention strategies ● Maintain knowledge and expertise in eligibility, enrollment, and program specifications for the PACE Program ● Support business development and census growth by participating and assisting with outreach programs as needed Schedule and Shift Details ● M-F, 8a-5p. Mostly in-field home appointments and occasional onsite work in Los Angeles clinic up to 5 days per month. Qualifications
- Associate degree or higher

3+ years of experience in ANY of the following:
- Geriatric Care
- Senior Care

ALL of the following valid licenses/certifications:
- Registered Nurse (RN) in California (CA)
- American Heart Association Basic Life Support (AHA BLS)
- CA driver’s license, personal transportation, good driving record and auto insurance
- RN (3+ years)
