Qualifications

Required
  • Associate degree or higher
  • 1+ years of experience in ANY of the following:
    • Hospice
    • Palliative Care
    • Oncology
  • ALL of the following valid licenses/certifications:
    • Registered Nurse (RN) in Ohio (OH)
    • American Heart Association Basic Life Support (AHA BLS)

Overview

As a RN Hospice Case Manager at our client, you will provide compassionate care to patients in need, utilizing your nursing skills and expertise to make a difference in their lives. This full-time position offers a rewarding opportunity to work with diverse populations and contribute to the well-being of our community. This positions supports the East side of Cleveland, OH. Key Responsibilities ● Provides direct care services to patients/families in community and institutional settings ● Utilizes principles of the nursing process to assist in data collection and evaluation of nursing care ● Communicates and collaborates effectively with health care providers, third parties, patients, and families based on specific role ● Maintains awareness of comfort and safety needs of patients ● Demonstrates nursing knowledge and skills in the implementation of prevention, therapeutic and rehabilitative nursing care. ● Uses oral and written communication to convey pertinent information to members of the health care team in a timely manner. ● Responds to life-saving situations based upon nursing standards, policies, procedures, and protocol ● Coordinates communications with third party payors for pharmacy, DME, and other supplies needed for the clinical plan of care. ● Maintains knowledge of regulatory, licensing and accreditation agencies. Schedule and Shift Details ● Full-time position with a schedule of 7 days on, 7 days off from 4:30 pm-midnight, on-call between midnight-8:30 a.m., with weekend coverage between 8am-5pm Saturday and Sunday.
