Qualifications

Required
  • Associate degree or higher
  • 3+ years of experience in ANY of the following:
    • Inpatient Facility
    • Medical-Surgical (Med-Surg)
    • Medical/Surgical (MedSurg)
  • Valid Registered Nurse (RN) in Pennsylvania (PA)
  • ANY of the following valid licenses/certifications:
    • American Red Cross Basic Life Support (ARC BLS)
    • American Heart Association Basic Life Support (AHA BLS)
  • Have you spoke with a Relode Recruiter about this role via phone or video?

Overview

As a Home Recovery Care Registered Nurse, you will provide hospital-level care directly to patients in their homes based on the Admitting Provider’s Treatment Plan. You will participate in interdisciplinary care teams, administer top-quality care, and engage in patient-centered recovery support in the home setting. Key Responsibilities ● Participate in the Admission Huddle with the Admitting Provider and Recovery Care Coordinator ● Confirms Admission Health and Home Assessment findings upon initial visit to patient’s home ● Carries out physician’s plan of treatment and reports changes as necessary ● Assesses patient’s condition during home visits and charts observations ● Provides in-home nursing services including head to toe assessment, medication reconciliation, initiate IV and site care, basic wound care, lab collection, catheter care, and patient education ● Educates patients and their families on proper home health care strategies and procedures ● Prepares and submits clinical documentation outlining services rendered and patient's progress towards goals ● Documents and promptly communicates all provider order changes to the Recovery Care Coordinator ● Promptly communicates any patient changes to the Admitting Provider and Recovery Care Coordinator ● Initiates the Escalation Protocol as needed for changes in patient condition or status ● Participates with the discharge planning process and identify any needs as part of the care plan ● Instructs and supervises the patient and/or other responsible caregiver in meeting nursing and related patient care needs ● Participates in daily virtual huddles with Admitting Provider, RCC, and patient to discuss patients’ progress and Care Plan ● Provides patient education regarding the telehealth system and kit ● Utilizes time management and critical thinking skills at all times Schedule: Monday – Thursday 11am – 9:30pm (4 – 10 hour days)
