Key Responsibilities ● Provide management and delivery of patient care plans in facilities and communities ● Collaborate with the care team ● Support patients in Lisle, IL and surrounding areas Schedule and Shift Details ● Mon-Fri; 8A to 5P. Qualifications

Required
  • Bachelor's degree or higher
  • 1+ years of experience in ANY of the following:
    • Hospice
    • Palliative Care
    • Home Health
  • ALL of the following valid licenses/certifications:
    • Nurse Practitioner (NP) in Illinois (IL)
    • Registered Nurse (RN) in Illinois (IL)
  • ANY of the following valid licenses/certifications:
    • American Heart Association Basic Life Support (AHA BLS)
    • American Heart Association Advanced Cardiac Life Support (AHA ACLS)
  • Clinical Experience (5+ years)
  • Have you applied to or worked for Residential Healthcare Group anytime in the last 6 months?
  • Have you spoken with your relode recruiter about this role?
