Overview
Our client is seeking an experienced Nurse Practitioner to join their PACE Program in Mishawaka, IN. The ideal candidate will have at least 5-7 years of NP experience, with a focus on long-term care and working with the elderly population. Key Responsibilities ● Provide primary medical management in collaboration with the PACE Organization (PO) Physician(s) ● Perform assessments in collaboration with the physician and as needed ● Develop and implement appropriate plans of care to PO participants ● Evaluate participant physical complaints and provide appropriate treatment ● Provide participants and caregiver teaching and education ● Function as a member of the Interdisciplinary Team (IDT) ● Provide daily oversight of PACE Organizations Clinic ● Perform rounds in the POs contracted nursing homes ● Regularly inform the IDT of the medical condition of each participant, remaining alert to pertinent input from other team members, participant’s caregivers, as well as documenting changes in a participant’s medical record consistent with documentation policies Schedule and Shift Details ● Full-time, day shifts
Qualifications
- Master's degree or higher

ALL of the following valid licenses/certifications:
- American Association of Nurse Practitioners (AANP)
- American Nurses Credentialing Center (ANCC)
- American Heart Association Basic Life Support (AHA BLS)
- CSR/CDS
- Drug Enforcement Administration (DEA)
- Valid Nurse Practitioner (NP) in Indiana (IN)
- NP Long-Term Care (5+ years)
- Working w/ Elderly Patients (1+ years)
