LVN - Home Care Coordinator

Overview

Our client is seeking a Home Care Coordinator to develop and implement home care services for PACE program participants. The ideal candidate will have a passion for serving high-risk seniors and frail older adults, possess an LVN License, and have experience in managing home care services. Key Responsibilities ● Coordinate Home Care Services for participants using home care agencies ● Participate as a member of the Interdisciplinary Team (IDT) ● Conduct nursing care within LVN Scope of practice ● Provide general nursing care in the clinic and participant’s home ● Document in the EMR within set documentation timeline ● Communicate with home care employees to understand participant’s condition ● Collaborate with the Transportation Coordinator for participant transportation ● Schedule Home Care appointments and support scheduler for other appointment needs ● Oversee visits completed by external agencies Schedule and Shift Details ● Monday to Friday, 8a-5p, on-site. Qualifications

• Associate degree or higher
• 3+ years of experience in ALL of the following:
  • Home Health Care
  • Geriatric Care
• Valid Licensed Vocational Nurse (LVN) in California (CA)
• ANY of the following valid licenses/certifications:
  • American Heart Association Basic Life Support (AHA BLS)
  • American Red Cross Basic Life Support (ARC BLS)
• Proof of a valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law
