Key Responsibilities ● Collaborate to provide management and delivery of patient care plans ● Support patients in Springfield and surrounding areas ● Meet critical deadlines for charting ● Self-manage time and schedule ● Drive to/from patient locations Schedule and Shift Details ● Monday through Thursday 4 PM until midnight Travel ● Occasional local travel within the specified territory

Qualifications

• Associate degree or higher
• Hospice (2+ years)
• 1+ years of experience in ANY of the following:
  • Electronic Medical Record (EMR)
  • Other EMR
  • TruChart EHR
• Valid Licensed Practical Nurse (LPN) in Illinois (IL)
• Do you have a valid Driver's License, Reliable Mode of Transportation, and Auto Insurance (all 3)?
• Have you spoke with a Relode Recruiter about this role (via phone)?
• Are you okay with the required travel for this position?
• Have you applied to or worked for Residential Healthcare Group anytime in the last 6 months?
