Crisis Therapist
Overview
As a Crisis Therapist at our client, you will be responsible for providing crisis intervention and intensive case management services to adults with serious mental illness and/or co-occurring substance use disorders. Your role will involve conducting thorough assessments, deescalating crises, facilitating engagement in treatment, and preventing the need for higher levels of care. You will work closely with individuals and their families to ensure access to services and benefits, while maintaining an acute awareness of available supports within and outside the agency. Key Responsibilities ● Conduct screenings and comprehensive assessments to identify areas of need and service preferences ● Provide clients, guardians and/or family members with complete and accurate information regarding services and community resources available ● Assist clients in determining their eligibility for entitlements, community services and supports ● Coordinate with treatment team members and other providers to facilitate successful transition to less intensive services in the community ● Facilitate access to aftercare appointments following discharge from inpatient and residential treatment settings ● Provide transportation assistance as needed until the client is able to access transportation resources independently ● Develop Individualized Plans of Service with clients and other members of their treatment teams ● Provide face-to-face in-person office and/or home visits and/or telehealth services to each assigned client based on their needs and preferences ● Conduct community outreach as needed in order to facilitate client engagement or reengagement and to ensure access to needed services and supports ● Provide crisis intervention, crisis and safety planning as clinically appropriate ● Establish and maintain collaborative relationships with local providers ● Participate in regularly scheduled clinical supervision, team meetings, case consultations and huddles as scheduled ● Document all service activities and phone contacts within expected timeframes ● Perform within acceptable standards of productivity ● Maintain appropriate state licensure and certification ● Make referrals to Recipient Rights, Adult and Child Protective Services as required ● Complete 24 hours of program and/or position specific training annually ● Follow our client’s professional code of ethics ● Abide by all of our client's policies and procedures ● Other duties as assigned Schedule and Shift Details ● Regular working hours are Monday through Thursday 8:30 am to 7 pm, Friday 8:30 am to 5 pm and Saturday 9 am to 1 pm. Travel ● Local travel required

- Master's degree or higher
- Social Work (1+ years)
