Director of Case Management Houston, Texas Full-time Looking for a role in management or leading a team? Are you skilled in problem solving and thrive on managing many responsibilities? We have a director of case management position that could be yours! This job is for someone who will confidently and carefully lead the operations of the hospital social work and case management. This person will create and maintain effective systems to make sure all operations are running smoothly. Reports to Chief Financial Officer and is responsible for planning, organizing and directing Hospital Social Work and Case Management, Pre-Access programs in accordance with the federal, state and local regulations. Determines scope and emphasis of the programs, and maintains cooperative working relationships with other hospital departments and outside agencies in order to coordinate provision of health and medical care services. Guides program of staff development and ensures staff awareness of available community resources.
