Physical Therapist
Overview
As a Physical Therapist for our client, you will play a vital role in creating personalized care plans and delivering life-changing therapeutic care to older adults. Your expertise will help participants regain mobility and independence, while educating them and their families for continued progress at home. Key Responsibilities ● Participate in interdisciplinary team (IDT) meetings to complete assessments, develop individualized care plans, and review service determination requests. ● Evaluate mobility levels, physical conditions, and functional abilities to create tailored therapy plans. ● Deliver therapeutic exercises, gait training, and ambulation support to help participants achieve their goals. ● Develop engaging individual and group exercise programs focused on maintenance therapy and rehabilitation. ● Provide physical therapy services aligned with physician orders and the participant's care plan, while documenting interventions accurately. ● Conduct thorough assessments to identify priority areas and recommend follow-ups when needed. ● Recommend referrals for participants who may benefit from additional services or resources. ● Offer education to participants and their families, equipping them with the tools and knowledge needed for successful rehabilitation. ● Assist with tracking, managing, and maintaining therapy equipment to ensure participant safety and program efficiency. ● Work closely with staff in nursing homes, skilled nursing facilities, and assisted living settings to oversee rehabilitation needs for participants. ● Provide approvals for care plans in nursing home settings on behalf of the IDT. ● Conduct home assessments and deliver in-home treatments to meet participants where they are most comfortable. Qualifications
- Master's degree or higher

ALL of the following valid licenses/certifications:
- PT License in California (CA)
- American Heart Association Basic Life Support (AHA BLS)
- Physical Therapist (1+ years)
- Proof of a valid California driver’s license, personal transportation, a good driving record, and auto insurance as required by state law
