Qualifications

Required
  • Doctorate degree
  • 1+ years of experience in ANY of the following:
    • Internal Medicine
    • Family Practice
    • Occupational Medicine
  • Valid Medical Doctor (MD) in Oregon (OR)
  • Valid Board Certified
